All Applications should include the following information: 1. Name of Council 2. Name, address, and telephone number of president 3. Number of council members 4. Percent of members involved in the project 5. Dates of the project 6. Goals of the project 7. Audience for the project - who, number of participants 8. Description of the project (include pictures, publicity, other materials) 9. Evaluation of the project - refer to goals 10. Influences the project had on the council
